We are currently in search of a Home Health Care Coordinator that provides administrative services to our traveling nurses that provides in-home medical services to our patients. The Home Health Care Coordinator is to advocate for patients to make sure they get high-quality care. They perform a wide range of administrative tasks, such as entering patient data, answering phone calls and scheduling patient visits. This person will also assist in recruiting nurses for the regions. The role involves a significant amount of communication with patients, their family members and various Healthcare professionals. The Home Health Care Coordinator synthesizes information from these people, such as patient needs and healthcare goals, in a way that’s useful and helpful to the patient and Healthcare professionals.

Requirements:
- High school diploma required. AA or BA preferred.
- Minimum of 1-3 years of working in home health care or a medical facility.
- Multi-tasked.
- Good command of English oral and written. (Bilingual Spanish preferred).
- Knowledge of Microsoft Office or other computer systems required.
- Knowledge of EMR. (E-Clinical Works or similar preferred).
- Knowledge of medical billing, healthcare insurance.
